Startup Forms and Student Verification Forms were sent home electronically on Monday, September 19, 2022.

Medical Forms

Does your student have a prevalent MEDICAL CONDITION?

All students with prevalent medical conditions (i.e., anaphylaxis, asthma, diabetes, epilepsy) are required to have a Plan of Care and to have that document reviewed annually.

If you are new to Nelson this year (this includes all LEAP and grade 9 students), have a new diagnosis, or if there are changes to an already existing Plan of Care, please complete the appropriate form(s) below:

• Anaphylaxis Plan of Care Form

• Asthma Plan of Care Form

• Diabetes Plan of Care Form

• Epilepsy and Seizure Disorder Plan of Care Form

• Generic Plan of Care

If applicable, please also note:

• Appendix A (Form 1) - Request for School Personnel to Administer Prescribed Medication

• Appendix B (Form 2) - Authorization for Self-Administration of Prescribed Medication by Student

• Prescribed Medicines Administrative Procedure – Flow Chart

For students returning to Nelson this year, and if there are NO changes to your student’s Plan of Care that we have on file from last year, please complete only Appendix C. This is required from all students with an existing Plan of Care.
